Please fill out the vaccine registration form https://bit.ly/flacsvaccinepopup for each person that will be getting vaccinated. This is the form that the medical staff will use for your appointment.
Anyone 12 and up can come and will receive the Pfizer vaccine. Please know that anyone means: FLACS families (mom. dad, stepmom, stepdad, grandparents, aunts, uncles, FLACS student 12 and over, siblings 12 and over, cousins 12 and over).
FLACS Vaccine Day will be hosted in the FLACS MS building in collaboration with Bronx Rising Initiative (BRI) and Morris Heights Health Center (MHHC)
Date: Tuesday 7/6/2021. 9 AM - 4 PM
Location: FLACS MS 316 E 165th Street, Bronx NY 10456 (between Teller & Findlay Avenues)
